2. That the old address of Jesus Apostolic Church Ministries is No.254
Commonwealth Avenue, Old Balara, Quezon City, in which the said address
was later renamed as No.254 Don Mariano Marcos Avenue, Barangay Old
Balara, Quezon City.
3. That the church of Jesus Aposolic Church Ministries, Inc. was relocaited
at No.9600 St. Catherine Street, Capitol Parkland, Caloocan City in 1996,
from its former address at No. 254 Commonwealth Avenue, Old Balara
Quezon City.
4. That, until now, I am still ho;ding office at No. 254 Don Mariano Marcos
Avenue, Barangay Old Balara, Quezon City in which its former name is No.
254 Commonwealth Avenue, Old Balara, Quezon City.
5. That the receipt issued to maria Teresa F. Concepcion was printed on 1995
and still bear the office's old address.
